Comment
Hence, a renal biopsy was performed. A fragment of renal cortex containing 21 glomeruli was available for light microscopic examination. The light microscopic changes detected by Steven Emancipator are depicted.

Figure
1: Focal and segmental increases in mesangial
matrix and mesangial cellularity were present in 17 of the 21 glomeruli
in the biopsy; one glomerulus (not depicted) showed total global glomerulosclerosis. |
Figure
2: A small non-circumferential florid crescent
occupied part of the urinary space in 1 of 21 glomeruli. |

3: Small segments of sclero-hyalinosis with flocculent capsular
adhesion were present in 2 of 21 glomeruli. |

4: By immunofluorescence, intense (+++) IgA was deposited
in mesangial areas with some extension to subendothelial areas; although
not shown, there was associated C3(+++) and IgG(+) as well. |
