The problem with aminoglycosides is of course their nephrotoxicity and ototoxicity, especially in a patient with renal failure. We wanted to reach appropriate peaks as well as throughs, and this on a daily basis. We had the following options. Which schedule can be expected to give the most appropriate results pharmacokinetically?
A) Administer the aminoglycosides after dialysis each time through levels decrease below threshold.
B) Administer the aminoglycosides after dialysis aiming at regular threshold peaks and not taking into consideration through levels.
C) Administer the aminoglycosides thrice weekly before a regular standard hemodialysis session.
D) Administer the aminoglycosides daily before a shortened regular dialysis session.
E) Administer the aminoglycosides daily before a prolonged high-efficiency dialysis session.
F) Discard the idea of giving aminoglycosides.
